New Patients

If you are a new patient to our office and will be using your dental benefits, please provide the following information when requesting an appointment. In order to verify your benefit eligibility with our office, we will need the insurance company name, phone number, member ID number and group number and date of birth. It is always recommended to contact your insurance carrier directly to verify we are an in-network provider and ask any questions you have regarding your benefits. Your contract is between you and your insurance provider. We complete and send your insurance claim along with any diagnostic information on your behalf when treatment is completed at our office.
